• No-show/late cancellation policy: Please review and sign this form prior to your child’s first appointment to acknowledge this policy.
• Permission to treat: Fill this out if you expect someone other than a legal guardian to bring your child to an appointment.
• Registration form: Please fill out this form prior to your child’s first appointment.
• Release of information: This form authorizes the mutual exchange of your child’s medical records between Mary Bridge Children’s and another doctor’s office.
