A referral from your child’s MultiCare provider is required to receive services or make an appointment with this clinic. Once we receive a referral, our nursing staff will review to determine the appropriate next steps and resources.
Doctors may make a referral by faxing 253-864-3939. Any additional documentation, such as forms, may be faxed to 253-864-2778.
